Do you learn from accidents or from what goes well? - Workshop in oslo 29-30 January 2020

This workshop is based on the Costa Concordia case and is all about learning. Open up to different perspectives, be curious and engage in the discussions.

Day 1 with Nippin Anand PhD (Learning from accidents: The Costa Concordia case)
Day one will present a systematic analysis of the events leading up to, during and after the tragic accident of Costa Concordia. A key feature of this workshop is that we get to hear the perspective of the captain of the ship who was involved in the accident.

We will look at some underlying problems with our current approach to crew (bridge) resource management and will focus on team dynamics in safety critical operations. In the afternoon we will shed light on how and why organisational plans (i.e. emergency response and contingency manuals) are a serious disconnect from the ability to recover from failures.

Day 2 with Erik Hollnagel PhD (Learning from what goes well: Safety II in Practice)
Day two focuses on learning from what works well. This is about the distinction between Work-as-Imagined, which assumes that if the correct standard procedures are followed, safety will follow as a matter of course, and Work-as-Done, which appreciates how people at the sharp end of organisations learn to create safety in their work through performance adjustments that are not only desirable but essential.

While the use of off-the-shelf approaches may seem attractive in the short term this is usually ineffective in the long term. Knowledge, methods, and training must be tailored to an organisation’s purpose and practices in order effectively to address the bumps that are inevitable on the road to new practices. The purpose of day 2 of this workshop is to introduce the principles and practices that can help to do so.
